SKILLED NURSING
Skilled nursing services include dedicated care for complex medical issues in the comfort of the home. If you or a loved one is recovering from surgery, dealing with an injury, or requires care for a chronic condition, Fidelity Health Care Services has trained and dedicated medical professionals ready to help.
Fidelity Health Care Services has a pool of skilled nurses who are caring Registered Nurses or Licensed Practical Nurses. The “Plan of Care” is discussed with you, the physician following your care; and it is designed with your goals and needs in mind. We provide ongoing education you’ll need for when you or your loved one reaches a sufficient level of independence and our home health care services are no longer needed.
PHYSICAL THERAPY, OCCUPATIONAL THERAPY and speech THERAPY
Physical Therapy (PT) is for patients with lower body function issues; from strokes, post-operative spine, hip, knee, foot and ankle surgeries or generalized weakness. They will be focusing on strengthening, balance, and mobility.
Our occupational therapists focuses on improving your fine and gross motor skills so that you can carry out specific day-to-day activities.
Speech therapy is the assessment and treatment of communication problems and speech disorders. This may also include retraining of swallowing function of an injury or medical condition.
Maintaining mobility is important for people of all ages. After a serious injury or illness many people find it difficult to resume their regular daily tasks. Advancing age can also reduce activity, making it harder to keep doing the things we love. Based on your specific health needs, our therapists will develop a custom program of treatment including manual manipulation, physical exercise, traction, and other treatment modalities. Your therapy will be personalized to help you achieve treatment goals by reducing pain, improving recovery, and maintain and increasing mobility. Education and lifestyle counseling will enabling you to speed recovery and foster a healthy and active lifestyle.
On the first visit, your therapist will do a functional assessment to understand your strength and functional mobility. This will identify what your needs are as well as your goals and provide an honest estimation of what’s realistic. At that time your therapist will make a recommendation on the frequency of visits that will be planned.
Medical Social Work
Medical social workers provide a comprehensive assessment of the patients’ full range of needs and help them access the services and resources to improve their quality of life. This could include financial or community assistance such as transport and home meal delivery.
Our medical social workers collaborate with the patients, their family, and their healthcare team to create an individualized plan for the them especially if they have just been discharged from the hospital or any in-patient facility. They work quickly to identify problems and create immediate and longer-term interventions that the patients need to stay healthy and happy in the comfort of their own home and community.
HOME HEALTH AIDE
Home health aides provide crucial support which allows people to remain home when recovering from serious injury or illness or when dealing with a long term medical condition. Disability, illness, and injury can make it difficult to perform essential daily tasks. Home health aides perform necessary tasks including bathing, dressing, grooming, oral care, and toileting. Aides make sure that daily routines continue and basic care is not neglected.
For individuals suffering from a chronic disease, regular home health care can reduce hospitalizations and maintain health levels. During recovery from injury or surgery, assistance from a home health aide allows your loved one to focus on getting rest and recuperating. Home health aids also help manage regular exercise routines to build strength and maintain health.
